Membership Information
Our County Policy includes having all enrollment forms must go through club organizational leaders
This helps 4-H families, Club Leaders, and the Extension Office to all be on the same page.
While enrollment forms are made available with 4-H Club Leaders, some standard forms are available here.
Remember: For your enrollment to be complete, you must submit an enrollment form and an event release form.

Enrollment Forms & Important Information
- Adult Volunteer Application Packet
(Includes $5 fee, check or money order; paid to RACES)
- Adult Volunteer - Working With Minors Training Access Instructions
All 4-H Volunteers are required to complete the training, 4-H Working With Minors, each year prior to working with youth. Annual training renewal will begin each October 1st when enrollments begin. All 4-H Volunteers must have successfully completed training to be eligible to register for any 4-H event. Use attached instructions to access and complete the online training option.

To be completed by Club Organizational Leaders annually, and submitted to the Rio Arriba CES Office
- Rio Arriba County 4-H Fundraiser, Sponsorship, & Activity Request Form
Clubs are required to submit a 4-H Fundraiser, Sponsorship, & Activity Request at least Two (2) weeks prior to event.
